Q. A 68-year-old man presents with palpitations and mild dyspnea. ECG shows an irregularly irregular rhythm with no discernible P waves. Which of the following is the most likely diagnosis?

A) Atrial flutter

D) Ventricular tachycardia
E) Sinus arrhythmia

Answer: B — Atrial fibrillation. The hallmark ECG findings are an irregularly irregular rhythm with absent discrete P waves, replaced by fibrillatory activity. Atrial flutter classically shows a regular “sawtooth” pattern, and multifocal atrial tachycardia shows at least three distinct P-wave morphologies — both features not present here.
